Ces impulsions traversent le cr\u00e2ne et g\u00e9n\u00e8rent de faibles courants \u00e9lectriques dans les couches superficielles du cerveau. Dans le traitement de la d\u00e9pression, la stimulation est g\u00e9n\u00e9ralement dirig\u00e9e vers les r\u00e9gions pr\u00e9frontales impliqu\u00e9es dans la r\u00e9gulation de l&rsquo;humeur et le contr\u00f4le cognitif.<\/p>\n\n\n\n<p>\u00c0 notre clinique, <a href=\"https:\/\/www.neuro-outaouais.com\/fr\/smtr\/\">la SMTr<\/a> est pr\u00e9sent\u00e9e comme une option de traitement non invasive en psychiatrie pour des conditions incluant la d\u00e9pression r\u00e9sistante au traitement pharmacologique. Les s\u00e9ances durent habituellement de 40 \u00e0 50 minutes et sont r\u00e9p\u00e9t\u00e9es 5 fois par semaine pendant une p\u00e9riode de 4 \u00e0 6 semaines. Notre protocole n\u00e9cessite souvent une IRM structurelle pr\u00e9alable (non diagnostique) afin que notre \u00e9quipe puisse utiliser la neuronavigation lors de la planification. Cela nous permet d&rsquo;identifier avec pr\u00e9cision les sites c\u00e9r\u00e9braux \u00e0 stimuler et d&rsquo;orienter la bobine \u00e9lectromagn\u00e9tique sur ces cibles.<\/p>\n\n\n\n<p>La SMTr diff\u00e8re de l&rsquo;\u00e9lectroconvulsivoth\u00e9rapie (ECT). Elle ne n\u00e9cessite pas d&rsquo;anesth\u00e9sie et les patients demeurent \u00e9veill\u00e9s pendant toute la dur\u00e9e du traitement. Les impulsions provoquent des contractions des muscles du cuir chevelu, souvent ressenties comme un tapotement sur la t\u00eate, et certains patients rapportent un l\u00e9ger mal de t\u00eate. Aucun effet secondaire grave \u00e0 court ou \u00e0 long terme n&rsquo;est r\u00e9pertori\u00e9, et les traitements sont souvent suivis de s\u00e9ances de maintien \u00e0 intervalles variables.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Qu\u2019est-ce que la neuroplasticit\u00e9 ?<\/h2>\n\n\n\n<p>Le cerveau est un organe dynamique et non fig\u00e9. Il r\u00e9agit \u00e0 la stimulation, aux exp\u00e9riences et aux blessures en modifiant la communication entre les cellules et le fonctionnement des circuits neuronaux. Cette capacit\u00e9 d&rsquo;adaptation est ce que nous appelons la neuroplasticit\u00e9. Elle peut impliquer des changements dans l&rsquo;activation neuronale, la connectivit\u00e9 et la mani\u00e8re dont les diff\u00e9rentes r\u00e9gions c\u00e9r\u00e9brales collaborent. Il s&rsquo;agit d&rsquo;un processus normal du syst\u00e8me nerveux et d&rsquo;un \u00e9l\u00e9ment essentiel pour les cliniciens et chercheurs s&rsquo;int\u00e9ressant \u00e0 l&rsquo;apprentissage, \u00e0 la r\u00e9cup\u00e9ration et \u00e0 la r\u00e9ponse aux traitements des traumatismes cr\u00e2niens, des accidents vasculaires c\u00e9r\u00e9braux (AVC), des maladies neurod\u00e9g\u00e9n\u00e9ratives et des troubles mentaux (Fitzsimmons et al., 2024 ; Kricheldorff et al., 2022).<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Comment la SMTr peut-elle affecter les fonctions c\u00e9r\u00e9brales ?<\/h2>\n\n\n\n<p>La recherche d\u00e9montre que la stimulation magn\u00e9tique r\u00e9p\u00e9t\u00e9e semble produire plus qu&rsquo;un simple effet local passager durant la s\u00e9ance. Les donn\u00e9es actuelles sugg\u00e8rent qu&rsquo;elle peut influencer l&rsquo;excitabilit\u00e9 corticale et l&rsquo;activit\u00e9 des r\u00e9seaux neuronaux connect\u00e9s. Les chercheurs ont observ\u00e9 des changements dans la connectivit\u00e9 fonctionnelle, l&rsquo;imagerie structurelle, la modulation des neurotransmetteurs, la dynamique des r\u00e9cepteurs et les effets synaptiques. Globalement, ces travaux soutiennent l&rsquo;id\u00e9e que la SMTr peut influencer la fonction c\u00e9r\u00e9brale \u00e0 plusieurs niveaux (Fitzsimmons et al., 2024 ; Kricheldorff et al., 2022 ; Sharbafshaaer &amp; Cirillo, 2024).<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">L\u2019utilisation de la SMTr fond\u00e9e sur les donn\u00e9es probantes \u00e0 notre clinique<\/h2>\n\n\n\n<p>Plusieurs points sont d\u00e9sormais bien \u00e9tablis dans la recherche en neurologie. La SMTr est une option de traitement non invasive reconnue en psychiatrie. \u00c0 notre clinique, elle est offerte pour des conditions telles que la d\u00e9pression r\u00e9sistante, le trouble obsessionnel-compulsif (TOC) et le trouble de stress post-traumatique (TSPT). Ce qui demeure \u00e0 l&rsquo;\u00e9tude sont les d\u00e9tails plus pr\u00e9cis : le ciblage optimal, les param\u00e8tres de stimulation, les approches de maintien et les raisons pour lesquelles la r\u00e9ponse clinique varie d&rsquo;un patient \u00e0 l&rsquo;autre.
